Complete Your Paperwork
New patient forms will be available online soon. In the meantime, please arrive 15 minutes early and we'll get you taken care of at the office.
Meet Dr. Garland
Your first visit typically includes a comprehensive exam, digital X-rays as needed, and a full discussion of your dental health and any concerns you have.
Leave With a Plan
Dr. Garland will walk you through your findings and, if treatment is recommended, clearly explain all options and answer every question you have.
New Patient Forms
Online forms are coming soon. In the meantime, please arrive 15 minutes earlyto your first appointment and we'll have you complete paperwork at the office. Or call us ahead of time and we can email them to you.
Payment & Financing
Flexible Financing with CareCredit
We are a fee-for-service practice, meaning we collect payment at the time of service. We accept cash, check, and all major credit cards.
We also accept CareCredit — a healthcare credit card with flexible financing options that makes it easy to get the care you need now and pay over time.
While we are not in-network with any insurance plans, we will gladly provide you with a detailed receipt (superbill) so you can submit to your PPO insurance for potential reimbursement.
